Move functionality out of prepend.inc.php

prepend.inc.php really does a lot of different functions, it's a nice idea to split this functionality apart, and place it in appropriate areas.

I vote we don't touch 1.0 for this, since it's not a bug fix. 1.1.2 has my vote, just so it's out earlier than 2.0.
